How can I tell if I need Bio-Identical hormonal replacement therapy?
Simply put, the only way to tell you are hormone deficient is through testing and proper analysis of the results. Hormone balance cannot be accomplished with guesswork or based on insufficient information. It is critical to your diagnosis that your doctor order the correct lab work and understand what the results indicate—which is too often not the case. Once your hormone levels are accurately measured and analyzed, you will receive the proper dose of hormones to fulfill your body’s requirements. Your hormone levels will thereafter be tested periodically in order to assess the ongoing success of your hormone replacement therapy. Bio Identical Hormones (at times referred to us as Bio-Equivalent Hormones) are not to be confused with traditional hormone replacement therapy (HRT) that has received much negative press since the Women's Health Initiative (WHI) study was halted. The WHI study utilized commercial conjugated estrogens derived from the urine of pregnant horses, and Medroxyprogesterone (also known as progestins). Abruptly stopped in 2002, the WHI concluded that HRT increases a woman's risk for breast cancer and stroke. It is important to reiterate that the WHI was based on the use of commercial hormones that are foreign in structure to a woman's natural hormones. While in use for years, commercial/pharmaceutical hormones may have helped with menopausal symptoms because they can occupy hormone receptors in the female body. However, the body's ability to metabolize these hormones remains in question. The WHI study concluded that estrogen replacement therapy results in a slight increase in the chance of breast cancer, stroke, and uterine cancer.

More importantly since Bio-identical hormones are prescribed from compounding pharmacies, the dosage can be customized to meet an individual’s optimal dose. An individual's hormone pathways are similar to their finger print, unique to them.

What really is a midlife crisis?
Somewhere around our forty's something begins to change in our bodies. How are Bio-Identical Hormones most useful?
What is important to remember is that each individual has a genetic makeup that means their hormone systems and metabolic enzymes are distinct to them. Some of today's laboratory testing enables us to test the results of hormone metabolic pathways, as well as genetic markers to determine if a person has an increase genetic risk for hormone sensitive cancers. How can Bio-Identical Hormones be administered?
Bio-Equivalent hormone therapy, (sometimes referred to as Bio- identical hormone therapy), can be administered in several different ways. We prefer the injected method. This method has a smoother delivery of hormone. Hormone level testing is done before therapy to determine necessity, and every 6 weeks for the first 6 months, then every 6 months thereafter if levels remain therapeutic. Additionally, bio identical hormones can be administered through creams, gels, patches,and capsules.

.Other routes of administration
Oral hormone administration should be avoided in general, except in the case of progesterone supplementation. Oral estrogens or testosterone experience metabolic first pass effect and are broken down by the liver soon after ingestion. Topical administration of hormones avoids first pass metabolism by the liver, however topical administration can be messy, problematic and a daily affair. In the instance of topical testosterone application, an enzyme exists in the skin that can lead to undesirable side effects. In women, it can lead to undesirable hair growth.

Injections of testosterone can be given deep intramuscularly. The injection is given every 7-14 days. Typical form is Testosterone Cypionate, which is either in cottonseed oil or sesame seed oil. The injection technique may give a "peak-and-valley" of testosterone levels, with the peak coming immediately 24 hrs after the injection with a slow progression towards the "valley" over the course of the week. This method needs close supervision or, otherwise may not keep a static level and may result in symptoms returning towards the end of the week.
